IS&T Honors and Awards Nomination Form

To facilitate the selection of award recipients, it is important that the Honors & Awards (H&A) Committee have complete and accurate information. Please provide the requested information to the best of your abililty. We will collect additional information from nominees as needed.

Nominations are reviewed by the H&A Committee beginning in early December. The deadline for consideration is October 1st.
